If you don't spare in expenses: Get to Piazza di San Pietro by Via della conciliazone. men and women sell line-free ticket to the museum and the Cappella Sistina with tourguide included in the price
Better to make online reservations beforehand. The most useful excursion is Vatican's Gardens. It includes all possible museums/exhibitions.
Spend the extra money to get a semi-private tour and skip the insane lines. It is more than worth it.
You'll need luck to have a good guide... If unlucky, change the 'channel' on your audio device and follow a better guide. Also, it is better to bring your own earplugs.
